I'd love both simple star options as suggested by Tom, as well as the option for a custom text field. My organization has our own feedback surveys that are a little different depending on the event, so I'd love to be able to place those into templates and set them up to go out automatically after the completion of each event.

This is a great idea! Could be a simple star option (1 to 5 stars) and then an open text box that could send an email to the organizers with the feedback.
